3. Do any Exclusion Criteria apply?
- Transplant: General exclusion for all transplant patients
- OB: Pregnancy >20wks
- Cardiology: STEMI, Ablation, LVAD, Code PE, Packemaker implantation
- Oncology: Active treatment (Leukemia/BMT)
- Heme: Sickle Cell Crisis, Plasma Exchange
- Procedures: Pacemaker requiring MRI
4. Is the patient eligible for Home Hospital?
Diagnoses & Characteristics: CHF, COPD, Diverticulitis, PNA, UTI, Cellulitis.
Logistics: Patient lives in catchment area AND Home Hospital Nurse Navigator accepts.
